This kiosk and the associated white-labeled application(s) are provided as a convenience for LegUpRx guests to access third-party telehealth services via this app. Via this app, LegUpRx does not provide medical care, medical advice, diagnosis, treatment, or prescription services. Via this app, LegUpRx is not a health care provider and is not responsible for medical decisions, clinical eligibility, or treatment outcomes.
Leg Up Recovery ("LUR") does not provide medical services and is not a medical group. LUR provides technology, administrative support, and coordination services that allow users to connect with independent licensed clinicians through Beluga Health, P.A. and other affiliated professional medical entities ("Medical Providers"). All medical evaluations, decisions, diagnoses, prescriptions, and treatment plans are performed solely by these independent Medical Providers.
By scanning a QR code or requesting a link, you will be redirected to the Leg Up Recovery platform or its affiliated partners for medical intake. All clinical encounters, consent forms, telehealth disclosures, pharmacy information, and program terms are provided within those systems. Use of this kiosk does not create a provider-patient relationship with LegUpRx or Leg Up Recovery.
Via this app, LegUpRx does not collect, store, or have access to personal or medical information entered after scanning a QR code or opening a link. All personal and medical information is processed only by Leg Up Recovery and the independent Medical Providers in accordance with their respective Terms of Use, Privacy Policy, Notice of Privacy Practices, and Telehealth Consent.
Medication availability, pricing, packaging, compounding notices, and state-based restrictions are determined by Leg Up Recovery's pharmacy partners and Medical Providers. Compounded medications, when offered, are not FDA-approved, and all product-related disclosures are provided within the Leg Up Recovery intake flow.
